If you wish to join the Ambulance Provident Fund, you can apply by using the following online process below. Current members can also update their details either through our online Member Management System.

BENEFICIARY CLAIM FORM:

The Beneficiary Claim Form is to be used at the time of a members death. The form will need to be completed and signed by the nominated members' beneficiary and then submitted to the APF secretary. The APF is committed where possible to making beneficiary payments within 48 hours of being notified of the members death.

2024 Director Nomination Form - Nominations

2024 Director nominations - Open 01 October 2024: NOMINATIONS ARE NOW CLOSED.

This form is used to be nominated as a Director of the Ambulance Provident Fund for election at an Annual General Meeting. The form can only be submitted once the AGM is declared, must be signed by the applicant, nominator and seconder, and submitted to the Returning Officer by the prescribed date on the form.

Nominations for Director positions at the 2024 AGM are open from 01 October 2024. Completed nominations are to be submitted to the following email address by the nominated date advertised at the time on the form: AGM@ambprov.org.au

Vacant positions will be filled at the AGM.

In line with the constitution 1/3 of the available Board positions are elected each year. Members interested in applying for Director positions on the Board should complete and return the form available in this location by the date advised on the nomination form.

Note: Nominees must be registered with ASIC and be able to provide a current valid Directors ID number on the nomination form.

Proxy Vote Form

This form is used to provide a proxy vote to a member who is attending the Annual General Meeting or any Special Meeting. The form is modified as necessary to reflect specific agenda items. The form must be presented to the Secretary / Returning Officer before the commencement of the Meeting. The 'due by' dates are advertised on the form for those sending via email or post.
